Guzzardi is partnering with the Kosciuszko (Koz) Park Advisory Council to host our Annual Fall Family Health Fair on Saturday, November 4th at Koz Park, and we need your help to make the event a success! Use the form below to sign-up as a volunteer at our health fair to help bring critical services and resources to our community.

How to Volunteer:
To sign-up as a volunteer at our health fair please fill out the form below and Rep. Guzzardi's staff will be in touch with final details! Volunteer tasks during the Health Fair will include: table and chair set-up, hot apple cider and coffee distribution, traffic control, and community member sign-in.
Please Note:
If you can't volunteer for the entire event time, 7:45am - 12:00pm, that's alright! We welcome any support you can offer. Please note how long you can volunteer in the form below.

Guzzardi se ha asociado con el Consejo Asesor del Parque Kosciuszko (Koz) para organizar nuestra Feria Anual de Salud Familiar de Otoño el sábado 4 de noviembre en el Parque Koz, ¡y necesitamos su ayuda para que el evento sea un éxito! Cómo ser voluntario:
¡Para inscribirse como voluntario en nuestra feria de la salud por favor llene el siguiente formulario y el personal de Rep. Guzzardi se pondrá en contacto con los detalles finales! Las tareas de los voluntarios durante la Feria de la Salud incluirán: montaje de mesas y sillas, distribución de sidra de manzana caliente y café, control de tráfico y registro de miembros de la comunidad.

Si no puede ser voluntario durante todo el evento, de 7:45 a.m. a 12:00 p.m., ¡no hay problema! Agradecemos cualquier ayuda que pueda ofrecernos. Por favor, anota cuánto tiempo puedes ser voluntario en el formulario de abajo.
